This was a known failure for a particular run of these switches. They redesigned the switch and did replacements for a while but that was a few years back. You also have to make sure you don’t over torque the nuts when installing and make sure all four mounting lags are secure.
Even with proper installation all battery switches can fail. I always make sure I have a small pair of vise grips in my tool kit just in case so I can clamp two leads together.
